Description:
Participate in a survey study about changing your health habits and receive a $15 gift card.
We are looking for people who want to make healthy changes in the near future but who haven’t started yet to participate in a survey study.
If you intend to do any of the following:
- get into shape
- eat healthier
- stop unhealthy habits such as smoking
...then you may qualify.
We will ask you to complete a survey package which will be mailed to you. The survey takes approximately 30-45 minutes to complete.
